After you complete your green section, please submit it to your child’s school administrator.  Ask when you can return to pick up the finished document. 

 

 

School Administrator Instructions:

After receiving this form completed and signed by the parent, please complete the yellow section.    Once finished, return the completed form to the parent or guardian, or submit it for them using the Upload feature below or emailing it to contact@sanjoseon.com.  Thank you for your assistance.
